DHEA Sulphate Blood Test: Normal Ranges, Causes & What Your Results Mean

What Is DHEA Sulphate?

Dehydroepiandrosterone sulphate (DHEA-S) is the most abundant steroid hormone in the human body. Produced primarily by the adrenal glands, it serves as a precursor to both testosterone and oestrogen. DHEA-S circulates in the blood at much higher concentrations than DHEA itself because sulphation extends its half-life to 7–10 hours.

DHEA-S levels follow a distinctive lifetime pattern: they rise sharply during adrenarche (ages 6–8), peak in the mid-20s, and then decline steadily at approximately 2–3% per year. By age 70–80, levels may be only 10–20% of peak values. This decline has led researchers to investigate DHEA-S as a biomarker of biological ageing.

Because DHEA-S feeds into both androgenic and oestrogenic pathways, it plays roles in immune function, bone density, mood regulation, libido, and body composition.

Why Is DHEA Sulphate Tested?

  • Adrenal function assessment - DHEA-S is a direct marker of adrenal androgen production
  • Investigating androgen excess in women - elevated DHEA-S can cause hirsutism, acne, and menstrual irregularities
  • PCOS evaluation - approximately 20–30% of women with polycystic ovary syndrome have elevated DHEA-S
  • Adrenal tumour screening - markedly elevated DHEA-S may indicate an adrenal adenoma or carcinoma
  • Fatigue and low libido - low DHEA-S is associated with chronic fatigue, reduced vitality, and decreased sex drive
  • Ageing and longevity research - DHEA-S is increasingly used as a biological age marker

Normal Ranges

Group Normal Range (µmol/L)
Males 18–30 6.2–16.1
Males 31–50 3.4–12.2
Males 51–70 1.6–8.4
Females 18–30 3.3–11.2
Females 31–50 1.8–9.7
Females 51–70 0.5–5.6

DHEA-S is best measured in a morning fasting sample for consistency, although it is less affected by diurnal variation than cortisol.

What Do High DHEA Sulphate Levels Mean?

  • Polycystic ovary syndrome (PCOS) - adrenal-type PCOS with elevated DHEA-S
  • Congenital adrenal hyperplasia (CAH) - enzymatic defects in cortisol synthesis shunt precursors toward DHEA-S
  • Adrenal tumours - adrenal adenomas or carcinomas can produce excessive DHEA-S
  • Cushing's syndrome - ACTH-dependent forms may elevate adrenal androgens
  • DHEA supplementation - over-the-counter DHEA supplements directly raise DHEA-S levels
  • Stress - chronic stress activates the HPA axis, potentially elevating adrenal androgens

What Do Low DHEA Sulphate Levels Mean?

  • Age-related decline - the most common cause; DHEA-S naturally decreases with ageing
  • Adrenal insufficiency - Addison's disease or secondary adrenal insufficiency reduces all adrenal hormones
  • Chronic illness - prolonged illness, inflammation, and chronic fatigue syndrome are associated with lower DHEA-S
  • Long-term corticosteroid use - exogenous glucocorticoids suppress adrenal function
  • Anorexia nervosa - severe calorie restriction impairs adrenal function
  • Hypopituitarism - reduced ACTH secretion limits adrenal stimulation

How to Improve Your DHEA Sulphate Levels

  • Manage stress - chronic cortisol elevation can suppress DHEA-S production; practise mindfulness, yoga, or regular relaxation techniques
  • Regular exercise - moderate resistance training has been shown to support healthy DHEA-S levels
  • Quality sleep - DHEA-S production follows circadian patterns; prioritise 7–9 hours of uninterrupted sleep
  • Balanced nutrition - ensure adequate cholesterol intake (the precursor to all steroid hormones), healthy fats, and micronutrients including zinc and magnesium
  • Maintain a healthy weight — obesity alters the HPA axis and can affect adrenal hormone production
  • Discuss supplementation — DHEA supplements are available but should only be taken under medical supervision due to hormonal effects

When Should You Get Tested?

  • Women experiencing excess hair growth, acne, or irregular periods
  • Men or women with persistent fatigue, low libido, or low mood
  • As part of a hormone or biological age assessment
  • If adrenal insufficiency or adrenal tumour is suspected
  • Before and during DHEA supplementation to monitor levels
